Community Health Clinic Resources

Community Health Clinics are committed to providing care to all community members regardless of finances, insurance coverage, or ability to pay. They can provide a sliding fee scale based on family size and income. The clinics accept Medicaid, Medicare, CHP+, and most private insurance plans. They also have team members who can assist with insurance enrollment. 

Clinica Colorado: In response to the growing COVID-19 outbreak in our state, Clinica Colorado is offering NO COST virtual visits via phone or video call through April 30, especially for the uninsured. This service allows patients to receive the medical care they deserve, while maintaining the safety of all patients, families, staff, and the community.

Clinica Colorado launched CovidLine, a free hotline for COVID-19 screening and telehealth services for Coloradoans who do not have health insurance. CovidLine provides free COVID-19 symptom screening, phone-based appointments with volunteer doctors, and routing and recommendations for leveraging local resources. Local: 720.902.9449. Clinica Family Health: Offering telehealth care to patients. You can get medical, dental, and behavioral healthcare from the comfort and safety of your home. A telehealth visit is just like a regular visit between you and your provider, but instead of meeting in person, your visit is conducted over the phone. If your provider determines it is necessary based on your telehealth visit, we may ask you to come to the clinic for additional services. By using telehealth services, you can get the care you need while helping reduce the spread of the coronavirus by not coming into the clinic. How to Schedule a Telehealth Visit: You can schedule a telehealth visit just like schedule an in-person visit. Call us at 303.650.4460 and speak with a communications center specialist. Or click on “Request an Appointment” at our website. We will schedule a time for your provider to call you.

Paying for Telehealth: Clinica will continue to work with each patient to ensure access to services regardless of ability to pay.

Kids First Community Pediatric Clinics: Kids First Community Pediatric Clinic in Commerce City is OPEN. Additionally, the Brighton Clinic School-based health center has been opened as a community site for children ages 0-21. They are providing telehealth and in-person visits on Mondays, Wednesdays, and Fridays. The Commerce City Clinic also has been providing telehealth services five days per week. 

Due to construction at Gregory Hill ECC, the Westminster location is temporarily moving to the 84th Avenue Neighborhood Health Center at 2551 W. 84th Ave., Westminster 80031. The temporary location will be open Monday-Friday, from May 1 through July 31. Please call 303.428.4384 for more information.

Salud Family Health Center: If you are experiencing symptoms, please contact Salud Family Health Center over the phone at 303.697.2583 to speak to a member of the Care Team to determine further care. 
Screening Clients at Entry to Homeless SheltersThis is a screening tool used to identify people with symptoms that indicate they might have a respiratory infection. Although not every person who has respiratory infection symptoms will have coronavirus disease (COVID-19), using a tool may be helpful in identifying people who may need medical care. If someone screens positive for symptoms, they should be directed to where they can stay, either within the shelter or at another location, according to a predesignated plan developed with the local Continuum of Care (CoC), public health department, and community leadership.
Stride Community Health CenterHealth clinic that is currently providing telehealth medical and mental health services (via phone and/ or video), emergency dental, and COVID-19 testing for specific at-risk populations. They provide services to patients with Medicaid, private insurance, and patients with no current insurance. They are able to help patients apply for Medicaid or have a sliding scale fee for uninsured patients, depending on their family size and household income.
